1. Period of Initial Notification:
Maximum 30 calendar days from the date of the insured event.

2. The period of completing the Claim:
Maximum 90 calendar days from the date of the insured event.

3. Documents for filing a Death Claim:
a) Death Claim form;
b) Insurance Policy;
c) doctor's certificate regarding the cause of death (original);
d) proof of identity, in the form of Electronic Identity Card, Family Card, or passport for foreign citizens or other information issued by the authorized party (original or legalized copy) that applies on behalf of the Participant or designated party;
e) death certificate (original or legalized copy). If you die abroad, the authorized agency is the Consulate General of the Republic of Indonesia;
f) funeral or cremation certificate from the authorized agency (original or certified copy);
g) an accident certificate from the police (original or certified copy).

The company has the right to request other supporting documents that are relevant and reasonable.
